Ads not showing up on either Default or Custom Theme

Hello everyone, the website I am helping is a friends and I am trying to get this mod to work.

For some reason, it will not even work on the default page, here is the link:

also, I am using a theme called Black-n-Black is there a way to get this mod to work?  Thanks.
