Ads show up twice in ad management screen and report

For some reason, ads are showing up twice in the ad management screen and in the reports.  Attached is a screen shot.  Any help would be greatly appreciated.

Never mind, found the problem.  Duplicate entries in the index.php file.  Sorry for the trouble.

 After
install it I'm the one that make mistake and I
have try to contact some people for help
but no one help. It suppose to apear one

You have the system installed multiple times which causing it to display ads multiple times you need to uninstall completely. I may advise going to an earlier backup if possible.
